High and low level residential care
If you would like a residential place at Mayflower, and have been assessed as requiring either high or low level residential care, you must:
- Complete the Application for Respite Care or Permanent Entry to Aged Care Homes. This can be downloaded from the Department of Health and Ageing site click here.
- Have obtained Aged Care Assessment (ACAS) approval within the past 12 months. For more information click here.
- Complete a Mayflower application form, which you can obtain by telephoning Mayflower on 9592 2774 or download here.
Independent Living
To become a resident of Mayflower ‘s Independent Living Units (ILU) please complete a Mayflower Application for Independent Living Units which you can obtain by telephoning Mayflower on 9592 2774 or download here.
Fees and charges
Mayflower Retirement Community sets fees and charges for residential care according to the guidelines of the Commonwealth Department of Health and Ageing. Information regarding fees and charges can be accessed from the Department of Health and Ageing on the website click here or phone1800 500 853.
